SERVICE COORDINATION

Early intervention services are provided to help your child grow and develop, and to help you support and promote your child's development. These services based on your child’s needs include evaluation services (such as hearing and vision screening); home visits; speech, physical and other therapies; child development groups; family counseling; and, sometimes, even help with transportation. These services are provided at no out-of-pocket cost to you. The service coordination component primarily supports families in the Early intervention process from the stage of initial referral to planning the transition for eligible children to CPSE. PM Evaluation & Therapy Services offer Early Intervention services to children ages birth to three years old who have developmental concerns. These are home and community-based services and are provided at no cost to the family.

Services and supports are provided to families with infants and toddlers to maximize the child's developmental potential and can assist the family in helping the child develop and grow. The service planning and service delivery is primarily focused on meeting the unique needs of the child and family.


Where Do Families Start?

If you have concerns about the development of their child, please talk to your child's pediatrician as soon as possible. Any child from birth to age three with a developmental delay, disability or condition that affects the child's development may need help, please refer to the chart (pdf) provided Milestone Chart to monitor your child's growth. If you have concerns, please call the New York City Hotline 311 and ask for a referral to the New York City Early Intervention Program.

What Comes Next?

Once authorized by the NYC Department of Health, we will set up a developmental evaluation that will help determine eligibility and your child's unique needs. After the referral is made, and approved by the NYC Department of Health, the child is assigned a Service Coordinator who will guide the family through the process. The duties of the include; conducting an initial home visits to meet with the family, explaining the eligibility criteria, assist in identifying evaluation sites and facilitating the Individualized Family Service Plan (IFSP) meeting with the family and the NYC DOH Official where the mandates will be determined. The mandate of services includes the frequency and location where services will be provided. If the evaluation results show that your child is eligible, the evaluation team, the family and the NYC Department of Health Official (Early Intervention Official) will develop an Individual Family Service Plan (IFSP) that will address the families concerns, priorities, and outcomes.  The IFSP will build upon your family's routines & activities that are most important to you.  The Service Coordinator will complete necessary referrals for you to begin receiving Early Intervention Services.

Insurance and Other Services

PM Evaluation Service coordinators consist of a team of individuals fluent in Spanish, Bengali, Shona, Zulu, Urdu and Punjabi languages. The service coordinators will also offer guidance in acquiring other health-related services such as acquiring health insurance.
